Electronic Information System Attestations and Survey Requests

To streamline the process of collecting and evaluating electronic information system attestations, questionnaires and surveys, specifically focusing on the requirements for electronic information systems used in research, Research Compliance, in conjunction with Information Security, Information Management, and the Quantitative Science pillar, is here to assist our research community. Below, please find a detailed overview of our intake and review process to ensure your submissions of research data security questionnaires are handled efficiently and effectively.

Intake Process

  1. Submission: Research Teams may submit electronic information system attestations and survey requests through our online Airtable portal. Each submission should include all relevant documentation and a brief description of the research project, including the associated Cayuse/RASCAL#, if available. Please note that we must allow for 14 days for review and processing.
  2. Initial Screening: Our team will conduct an initial screening to ensure all required information is provided and that the submission aligns with policy compliance and Lurie Children’s infrastructures. We will also coordinate any authorizations that may be necessary.
  3. Acknowledgment: Once the initial screening is complete, researchers will receive an acknowledgment email confirming receipt of their submission, along with any notice of next steps as applicable.

Review Process

  1. Detailed Review: Our team reviews each submission in detail, assessing the relevance, completeness, and quality of the data provided.
  2. Feedback and Revisions: If necessary, researchers may be asked to provide additional information or make revisions to their submissions based on the team’s feedback.
  3. Final Approval: Once all criteria are met, the submission is approved, and researchers are notified of the outcome.
